Application Instructions

• Complete the online form/agreement:
  • Presentation title.
  • Select the Track/Subject area that best fits. Only select one.
  • Provide a short presentation summary in narrative form (20 words or less). This summary should state what the audience will learn/benefit from by attending the presentation. Remember to be clear and concise.
  • Provide short biographical sketches for both presenters (50 words or less for each presenter).
  • List 2-3 objectives for the presentation in bullet form.
  • Provide a concise description of your proposed presentation in narrative form (300 words or less). Please avoid acronyms or abbreviations.
• Biographical sketches, presentation summaries and abstracts of each accepted presentation will be reproduced in the conference notebook and on the CoSN website exactly as submitted. If you update this information prior to the conference, please provide this information to conference management.
• Disclose any organization or institution whose products or services will be discussed and for which there is a potential conflict of interest.
• Incomplete applications will not be accepted for consideration.
